Student Loan Forgiveness for Doctors and Healthcare Workers
Physicians, dentists, pharmacists, therapists and other healthcare workers often carry the largest student loan balances, and they also have more forgiveness and repayment programs than almost anyone else. The 2026 changes to loan limits and repayment plans shift the strategy, especially for new students. Here's what's available as of November 2026.
- PSLF is the big one: tax-free forgiveness after 120 qualifying payments while working full time for a nonprofit or government employer. Residency at a qualifying hospital counts.
- Federal loan repayment programs pay down debt in exchange for service: NHSC (up to $75,000 for two years of full-time primary care in the 2026 cycle), IHS (up to $50,000 for an initial two-year commitment), and NIH (up to $50,000 a year for research).
- New students face caps: professional programs like MD, DO, DDS and PharmD can borrow up to $50,000 a year and $200,000 total. Grad PLUS is gone for new borrowers.
- Anyone with a loan made on or after July 1, 2026 repays on RAP or Tiered Standard. Only RAP counts for PSLF.

PSLF for physicians and healthcare workers
PSLF forgives your remaining Direct Loan balance after 120 qualifying monthly payments made while you work full time (at least 30 hours a week) for a qualifying employer: government agencies, including public and VA hospitals, and 501(c)(3) nonprofits, including most nonprofit hospitals and academic medical centers. PSLF forgiveness is federally tax-free.
Why residency matters
Residency and fellowship years count toward PSLF if your employer is a qualifying hospital and you're on a qualifying plan. Because resident pay is relatively low, income-driven payments during training are low too. That's what makes PSLF so valuable for physicians: low payments early, larger forgiveness at the end.
Illustration (simple math, not a prediction)
On RAP, an AGI of $65,000 with no dependents falls in the 6% bracket: 6% × $65,000 = $3,900 a year, or $325 a month. Each on-time, full payment counts toward PSLF if you have certified qualifying employment. See how RAP is calculated, or compare plans with the IDR Payment Estimator.
Employer traps to check
- Your employer is whoever issues your W-2. Working at a nonprofit hospital doesn't qualify you if you're employed by a for-profit physician group or staffing company.
- States that bar hospitals from hiring physicians directly (California and Texas are the usual examples): since July 1, 2023, PSLF rules count certain contractors who provide services a qualifying employer can't legally fill with a direct employee under state law. Confirm with the PSLF Help Tool and your servicer before relying on it.
- The “substantial illegal purpose” rule that could have disqualified some hospitals providing gender-affirming care to minors was vacated by federal courts in June 2026 before it took effect. ED has appealed. See PSLF employer eligibility in 2026.
Certify employment every year and every time you change employers, including when you move from residency to an attending job. If you had months in forbearance during training, PSLF Buyback may let you count them later. Track everything with the PSLF Timeline Calculator.
Federal loan repayment programs
National Health Service Corps (NHSC)
For the 2026 cycle, NHSC offered up to $75,000 (full time) or $37,500 (half time) for primary care providers, and up to $50,000 or $25,000 for other eligible disciplines, in exchange for two years at an approved site in a Health Professional Shortage Area. Eligible disciplines include physicians, nurse practitioners, certified nurse midwives and physician assistants in primary care, dentists and dental hygienists, and licensed behavioral health providers. Eligible Spanish-speaking applicants could receive an extra $5,000. The 2026 cycle is closed, and HRSA hasn't announced the next one, so sign up for HRSA's alerts.
Indian Health Service (IHS)
IHS repays up to $50,000 of eligible health professions loans for an initial two-year commitment at facilities serving American Indian and Alaska Native communities, and participants can extend year by year. As of late October 2026, IHS was accepting applications for its FY2027 competition.
NIH Loan Repayment Programs
For researchers, the NIH programs repay up to $50,000 a year of qualified educational debt in exchange for a research commitment. The extramural cycle, for researchers not employed by NIH, opened September 1, 2026 and closes November 19, 2026, with awards starting July 1, 2027.
Stacking with PSLF
You can often pursue PSLF while working at a site that also qualifies for a repayment program, but a lump-sum award isn't the same as 120 monthly payments. Ask both the program and your servicer how the two interact for your loans before you sign a service contract.
New students: the 2026 borrowing limits
For students who weren't already borrowing for their program before July 1, 2026, Grad PLUS is gone. Professional students can borrow up to $50,000 a year and $200,000 total in federal loans, within a $257,500 lifetime cap. ED's interim professional list includes medicine (MD, DO), dentistry, pharmacy (PharmD), optometry, podiatry, physical and occupational therapy, physician assistant, audiology, speech-language pathology, chiropractic, MSN, DNP and nurse anesthesia programs. That list is still being litigated. Programs not on it are capped at $20,500 a year and $100,000 total. For the full picture, see Grad PLUS is gone.
Repayment changed too. Any loan first disbursed on or after July 1, 2026 makes you a “new borrower,” and once it enters repayment all your Direct Loans must use RAP or Tiered Standard. Tiered Standard doesn't count for PSLF, so if you're aiming for PSLF, choose RAP.
Taxes
- PSLF: federally tax-free.
- IDR forgiveness after 20, 25 or 30 years: taxable again starting in 2026. See the IDR tax bomb.
- Employer loan repayment: up to $5,250 a year tax-free under the employer educational assistance rules, which the 2025 law made permanent. Amounts above that are generally taxable wages.
- Repayment programs each have their own tax treatment. Read the program terms. Our nurses guide notes that Nurse Corps awards are taxable.
Medical residents and new attendings are heavily targeted by “physician loan forgiveness” services and refinance pitches. Refinancing federal loans into private ones permanently ends PSLF eligibility for that debt. Every federal application, from IDR and consolidation to PSLF, discharges and default resolution, is free at StudentAid.gov or through your servicer. Anyone who charges a fee, asks for your FSA ID password, or promises guaranteed forgiveness is a red flag. See how to spot student loan scams.
What to do this week
- Confirm your employer (the W-2 issuer) qualifies for PSLF with the PSLF Help Tool, and certify your employment.
- Make sure you're on a PSLF-qualifying plan: IBR, PAYE or ICR for older loans, or RAP. Not Tiered Standard.
- If you're a researcher, check NIH LRP eligibility now. The extramural deadline is November 19, 2026.
- Sign up for HRSA alerts for the next NHSC and Nurse Corps cycles, and check your state's loan repayment program.
- Ask your HR department whether your employer offers student loan repayment benefits.
FAQ
Does residency count toward PSLF?
Yes, if your residency employer is a qualifying government or 501(c)(3) hospital, you work full time, and you make qualifying payments on a qualifying plan. Low income-driven payments during residency still count.
How much does NHSC loan repayment pay?
In the 2026 cycle, up to $75,000 for two years of full-time primary care service in a Health Professional Shortage Area, or up to $50,000 for other eligible disciplines. Half-time awards are half those amounts. The 2026 cycle is closed.
How much can medical students borrow now?
Students in professional programs like MD and DO can borrow up to $50,000 a year and $200,000 total in federal loans, within a $257,500 lifetime cap. Grad PLUS is no longer available to new borrowers.
Can doctors in California or Texas get PSLF?
Possibly. Since July 1, 2023, PSLF rules can count certain contractors who provide services a qualifying employer can't legally fill with a direct employee under state law. Confirm with the PSLF Help Tool and your servicer.
Should I refinance my medical school loans?
Not if you might pursue PSLF or need income-driven payments. Refinancing federal loans into a private loan permanently ends PSLF and federal repayment protections for that debt.
